Taylor: But you won't be able to make a standard action against it.
Rachel: No need. I'll just slam my whole body into it as a free action.
Rachel: And with momentum being a thing, that should be enough to throw the megatank's aim off.
Taylor: Hmm. . . Roll for a Touch Attack.
Rachel: Gladly.

1. Generally you can't make a ranged AoO
2. Wasn't Taylor panicking that they were going to end the campaign too early? Wouldn't laser murdering Aelita be the same issue?

1. ... Crap. Didn't realize. Not much I can do about that since nearly every monster only has ranged attacks.

2. Yeah, but his top priority at the moment is Madeline. Taking this kind of action is unusually bold for her, and he wants her to know that she can play and do things her way even if it could lead to trouble. Taylor did give Madeline an out, but she wanted to go through with it, and he won't shoot down her idea because of that reason.

She could have been holding her turn,
Might be a class feature to take damage for an ally.
Could be the mechatank takes a turn to charge its attack and then shoot, giving her time to run forward in time to take the hit.

Also, there are usually feats that allow for a ranged AOO

i'd go with the assumption that the Megatank have some perk that allow them to make a exception.
As for killing her, well they could still reworks things into a "after the end" plot, where after failing to stop XANA, they have to organise a resistance against it. Also Taylor can't really have the enemy not attack in a situation where they can, should and have no reason not too.
